Carrier must provide the following:

• Signed copy of JBS Broker - Carrier Agreement *
• Insurance certificate naming JBS Logistics as holder *
• Auto & General Liability (at least $1,000,000)
• Cargo Liability (at least $100,000)
• Workman's Compensation (limits required by law)
• Copy of DOT Authority and Safety Rating (if assigned) *
• Signed W9 form returned *
• Completed carrier profile returned
• JBS Fast Pay Program Authorization Agreement (if applicable)

* Note: JBS will not provide payment to carrier unless information is provided.

JBS information includes the following:

• JBS Profile & History
• JBS Credit References
• JBS Surety Bond (AMTEX $100,000)
• TIA Performance Bond ($100,000)
• DOT Authorization (MC 194191)
• W-9 (Sign and Return)
• Broker - Carrier Contract
• Carrier Profile (Complete and Return)
• FastPay Agreement
